How BMI Is Actually Calculated — And Why It's Not the Whole Picture

Body Mass Index (BMI) is one of the most widely used health metrics in the world, appearing in everything from doctor's office charts to insurance calculations. It's also one of the most widely misunderstood — both in how it's calculated and, more importantly, in what it can and can't tell you.

The formula

BMI = Weight (kg) ÷ Height (m)²

Or in imperial units:

BMI = (Weight (lbs) ÷ Height (in)²) × 703

For example, someone weighing 70 kg at 1.75 m tall: 70 ÷ (1.75 × 1.75) = 70 ÷ 3.0625 = 22.9

The standard categories

  • Below 18.5: Underweight
  • 18.5–24.9: Normal weight
  • 25.0–29.9: Overweight
  • 30.0 and above: Obese

These thresholds were established by the World Health Organization and are widely used as a population-level screening tool — meaning they were designed to track health trends across large groups, not to serve as a precise individual diagnosis.

Why BMI was never designed for individual diagnosis

BMI was developed in the 1830s by Adolphe Quetelet, a Belgian mathematician — not a physician — studying the "average man" for population statistics, not individual health assessment. It only uses two numbers: height and weight. It has no way to distinguish between muscle mass and fat mass, no way to account for bone density, and no way to factor in where fat is distributed on the body (which matters significantly for health risk).

Where BMI genuinely breaks down

  • Athletes and muscular individuals: muscle is denser than fat, so a bodybuilder or athlete can register as "overweight" or even "obese" by BMI despite having very low body fat.
  • Older adults: muscle mass naturally declines with age while fat can increase, meaning two people with identical BMI can have very different actual body compositions.
  • Different body frames: BMI doesn't account for frame size — someone with a naturally larger bone structure can register higher without carrying more fat.
  • Pregnancy: BMI calculations aren't meaningful during pregnancy due to expected and healthy weight gain.
  • Ethnic variation in health risk: research has shown that health risks associated with a given BMI can differ across ethnic groups — some populations show increased health risks at lower BMI thresholds than the standard cutoffs suggest.

What BMI is still useful for

Despite its limitations, BMI remains a fast, free, and genuinely useful first-pass screening tool, especially at a population level or as one data point among several. It's inexpensive to calculate, doesn't require special equipment, and correlates reasonably well with health risk on average across large groups — it just isn't precise for any single individual, especially those in the edge cases above.

Better metrics, when available

For a more complete picture, health professionals often use BMI alongside:

  • Waist circumference — a strong independent predictor of health risk, since abdominal fat carries different risk than fat elsewhere on the body
  • Waist-to-hip ratio — another distribution-based measure
  • Body fat percentage (via skinfold calipers, bioelectrical impedance, or DEXA scans) — directly measures fat rather than inferring it from weight and height

The bottom line

BMI is a quick, useful starting point — not a diagnosis. If your BMI falls outside the "normal" range, especially at the borderline, that's a reasonable cue to look deeper (waist measurement, body composition, or simply a conversation with a doctor), not a verdict on your health. It's one number out of many that matter.
